An essential element of teaching is listening to the story data shares with us. Use this editable form to record data from the previous school year before you get to know students. Add new data throughout the year. Use data to provide students with the best instruction that meets their individual needs. Look at overall data to best meet the needs of your group as a whole.

Sticky Note Bookmarks

Students mark their reading with a sticky note using the bookmark prompts.

I love using these bookmarks to check in on student comprehension of their independent reading. Here's how they work: As students read 20 minutes each day, they mark their reading with one sticky note using the prompts on the bookmark for ideas and inspiration. In class, the teacher conferences with students asking them about their reading and to share their most recent sticky note.
